IRMAA is an income-related surcharge added to both Part B and Part D. It is based on the modified adjusted gross income reported on your tax return from two years earlier — 2026 premiums are set by 2024 income.

Married filing separately after living with a spouse during the year uses its own two-tier schedule: $83.30 Part D surcharge above $109,000, and $91.00 at or above $391,000.
IRMAA is a cliff, not a phase-in. A joint filer at $218,000 pays the standard premium. At $218,001 both spouses move a bracket — $1,948.80 more for the household over the year, triggered by a single dollar.
That is why a Roth conversion, a capital gain, or an RMD taken without planning can cost far more than the income tax on it alone. Model it with the IRMAA calculator before you take the income.

From $185.00 to $202.90 — an increase of $17.90 a month, or 9.7%. For context, the 2026 Social Security COLA is 2.8%, so the Part B increase consumed a meaningful share of the raise for beneficiaries whose premium is deducted from their check.
